These services assist individuals in gaining access to programs that will enrich their lives and foster individual growth, self-reliance and independence.
An assigned support coordinator for each family manages these services along with an Individualized Support Plan (ISP). This comprehensive and customized document coordinates and provides prior authorization for all New Jersey Division of Developmental Disabilities (NJ DDD) services and service providers. The support coordinator maintains the ISP, monitors budget for services and completes any and all required documentation.
